Chestnut Family Health Center is a community-based health center that offers adult and pediatric primary care.
The staff at Chestnut Family Health Center is committed to delivering comprehensive, high quality, personal care to our patients. The Center is a program of Chestnut Health Systems, which has been providing services throughout Illinois since the early 1970s. All of our physicians and advanced practice nurses are board-certified and are dedicated to providing medical care which is specifically tailored to your needs and the needs of your family.
Dr. Brockhouse, has practiced family medicine in Bloomington-Normal since 1991. Having earned two degrees from Illinois State University, she has called Bloomington-Normal home since 1976. She earned her medical degree from Chicago College of Osteopathic Medicine in June 1988, and went on to complete the SIU/Decatur Family Practice Residency Program in August 1991. Dr. Brockhouse is board-certified in Family Medicine with recent re-certification in April 2013.
Dr. Brockhouse provides care for individuals and families from infancy through the geriatric patient. She practices all aspects of family medicine from chronic disease management to acute illness/injury. Dr. Brockhouse’s interests are women’s health preventive medicine, and addiction medicine. She is delighted to be a member of the Chestnut Family Health Center team, and hopes to build a diverse practice with patients of all ages who want quality health care.
Kelly is a board-certified Family Nurse Practitioner with over 20 years of nurse practitioner experience. She completed her Master’s in Public Health from the University of Illinois in Chicago. During her career in Central Illinois, Kelly spent many years at Advocate Bromenn Medical Center in various roles and eight years at Planned Parenthood.
Michelle is a board-certified Family Nurse Practitioner with over 10 years of nurse practitioner experience. She completed her Masters in Nursing Science at Illinois State University, Mennonite College of Nursing. She is a member of the American Association of Nurse Practitioners. Michelle has worked in a variety of ambulatory settings with patients of all ages.
Beth is a Licensed Clinical Professional Counselor and an Art Therapist with over 20 years experience in providing counseling to Central Illinois patients. She received her Bachelors in Psychology from the University of Illinois and a Masters in Expressive Therapies from Lesley University in Cambridge, MA. Her passion is to help people achieve happy and healthy lives through building on their strengths.
Meg is a Licensed Clinical Social Worker with over 20 years of experience in social work. During her social work career she has worked with various populations in need of counseling and case management. Meg has a passion for integrating behavioral health services with primary care, and worked at a federally qualified health center in Washington, DC for seven years, working on integration and expanding mental health services. Meg earned her Master’s in Social Work from the University of Illinois at Chicago. Meg is responsible for coordinating the behavioral health program at Chestnut Family Health Center, which includes counseling, case management and community support.
Arif Habib, MD is a licensed physician and surgeon with a board certification in psychiatry. Dr. Habib received his medical degree from Dow University of Health Sciences and his board certification through the American Board of Psychiatry and Neurology, Inc. Dr. Habib has been practicing for more than 20 years with a focus on adult psychiatry. In addition to working at Chestnut Health Systems, Dr. Habib has a private practice and admitting privileges at both Gateway Regional Medical Center and Mercy Hospital.
Dr. Ather Malik is Board Certified in Family Medicine through the American Osteopathic Board of Family Physicians. He is a devoted and accomplished physician with over ten years in urgent care and clinical settings. He possesses a strong background with great experience in diverse and fast paced settings. He has a strong ability to connect and communicate, and establish professional rapport with patients, physicians, and staff members. Dr. Malik is a graduate of the New York Institute of Technology – New York College of Osteopathic Medicine. He is fluent in English and Urdu.
Jenia Heavens, BSN, MSN, FNP is a board-certified Family Nurse Practitioner. Jenia completed her masters in Nursing Science at Maryville University and her Bachelors in Nursing Science at Southern Illinois. She is a member of the American Association of Nurse Practitioners.
Paula Miller, MSN, PMHNP-BC has dual certification as Adult Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner. She received her Master of Science in Nursing Degree in the Nurse practitioner program in spring 2008 from Maryville University. Ms. Miller received her national certification as Adult Nurse Practitioner in 2008 from the American Academy of Nurse Practitioners currently known as American Association of Nurse Practitioners. Ms. Miller received her Post Master Graduate Certificate in Mental Health Nursing in spring 2015 from the University of Tennessee. Ms. Miller received her national certification as Psychiatric Mental Health Nurse Practitioner from American Nurses Credentialing Center in 2016. Ms. Miller has clinical experience in behavioral medicine, internal medicine, psychiatry, substance abuse, community health, and pain management. Ms. Miller is a health care professional with a commitment to providing excellent care.
Tracy Pacini, BSH, MSN, FNP is a board-certified Family Nurse Practitioner (FNP). Tracy is certified through the American Nurses Credentialing Center. Tracy completed her Masters in Nursing Science at University of Missouri- St. Louis. Ms. Pacini has been with Chestnut Health Systems since February 2015. She specializes in adult mental health, as well as child and adolescent mental health. Tracy enjoys working with a wide variety of patients. She is also employed by the University of Missouri- St. Louis as Adjunct Faculty since 2012. Ms. Pacini’s goal is to improve the life of those she works with and provide them with tools for success.
Jim Tarasuik, APRN is a Board Certified Family Nurse Practitioner. His focus is on providing comprehensive care across all age groups. His approach to care is concentrated on empowering individuals to recognize the daily challenges they face in managing their care. When these challenges are identified he helps foster a plan to successfully manage each concern. His attention is tuned to preventative care. He adeptly manages acute and chronic health concerns and disease processes with evidence based practice. His communication style with patients is one of openness, consistency and honest insight. Jim is a lifelong resident of Southern Illinois and graduate of Southern Illinois University Edwardsville. He has served in many diverse care roles during his 23 years in health care including service as an Officer in the United States Air Force. He continues to serve his community outside of the clinic by serving on various civic and community boards along with participating in many community and veteran organizations.
